What cause puffy ,sore ,bright red orifice?

my 17 yr old son get this from time to time!! thanks and sometimes a sweaty patch on his pillow xWhat cause puffy ,sore ,bright red orifice?
allergy to pepper like chili or green pepper
dehydration
allergy?
It could be an allergic antipathy to something that he ate.
too much kissing ,and the sweaty patch is him dreaming of kissing
dunno but i get them too, really annoying!
dust can impose this.
